All of the most widely-cited studies for the prediction of maximum exercise responses have utilized either volunteers or referred subjects. Therefore, selection bias, with overestimation of the reference values, is a likely consequence.In order to establish a set of predictive equations for the gas exchange, ventilatory and cardiovascular responses to maximum ramp-incremental cycle ergometry, this study prospectively evaluated 120 sedentary individuals (60 males, 60 females, aged 20±80), randomly-selected from >8,000 subjects. Regular physical activity pattern by questionnaire, body composition by anthropometry and dual energy X-ray absorptiometry (n=75) and knee strength by isokinetic dynamometry were also assessed.Previously reported equations typically overestimated the subjects' peak oxygen uptake (p<0.05). Prediction linear equations for the main variables of clinical interest were established by backward stepwise regression analysis including: sex, age, knee extensor peak torque, bone-free lean leg mass, total and lean body mass, height, and physical activity scores. Reference intervals (95% confidence limits) were calculated: some of these values differed markedly from those formerly recommended.The results therefore might provide a more appropriate frame of reference for interpretation of the responses to symptom-limited ramp incremental cycle ergometry in sedentary subjects; i.e. those usually referred tor clinical cardiopulmonary exercise tests.
The objective of the present study was to compare anthropometry with bioelectrical impedance (BIA) in relation to densitometry (dual-energy X-ray absorptiometry; DEXA) as methods of nutritional assessment and body composition in out-patients with chronic pulmonary obstructive disease (COPD). We conducted a cross-sectional clinical study with sixty-one patients with COPD (forty-two men and nineteen women), mean age of 66·5 (SD 7·9) years and forced expiratory volume in 1 s of 1·3 (SD 0·6) litres (52·2 (SD 19·8) % predicted), referred to the Pulmonary Rehabilitation Center. The patients were evaluated regarding nutrition status and body composition as determined by anthropometry, BIA and DEXA. In the results, 34·4 % showed mild obstruction, 31·2 %, moderate and 34·4 %, severe obstruction. According to the BMI (mean 24·5 (SD 4·5) kg/m 2 ), 45·9 % of the patients exhibited normal weight, while 27·9 % were underweight and 26·2 % were obese. Related to fat-free mass (FFM), anthropometry and BIA compared with DEXA presented high correlations (r 0·96 and 0·95 respectively; P,0·001) and high reliability between the methods (a 0·98; P, 0·001). Agreement analysis between the methods shows that anthropometry overestimates (0·62 (SD of the difference 2·89) kg) while BIA underestimates FFM (0·61 (SD of the difference 2·82) kg) compared with DEXA. We concluded that according to the nutritional diagnosis, half of our population of patients with COPD showed normal weight, while the other half comprised equal parts obese and underweight patients. Body composition estimated by BIA and anthropometry presented good reliability and correlation with DEXA; the three methods presented satisfactory clinical accuracy despite the great disparity of the limits of agreement.Chronic pulmonary obstructive disease: Nutritional assessment: Anthropometry: Electrical bioimpedance: Dual-energy X-ray absorptiometry
NAJAS, M.S. et al. Padrão alimentar de idosos de diferentes estratos socioeconômicos residentes em localidade urbana da região sudeste, Brasil. Rev. Saúde Pública, 28:187 -91,1994. Foram estudados 283 idosos do sexo masculino e feminino pertencentes ao projeto multicêntrico "Identificação de Necessidades dos Idosos Residentes em Zona Urbana do Município de São Paulo", estratificados por nível socioeconômico em três regiões do Município de São Paulo, SP -Brasil. Utilizou-se o método de freqüência de alimentos para se obter o padrão alimentar do grupo analisado. Os resultados indicam que no grupo de alimentos energéticos, mais de 90% dos indivíduos das três regiões ingerem feculentos, arroz, pão e macarrão; porém, apenas o arroz e o pão são utilizados diariamente. Quanto ao grupo de alimentos protéicos, 70% ou mais dos idosos consomem feijão, carne de boi, aves, leite e ovos, entretanto, no consumo diário, existe uma diferenciação entre as regiões analisadas. Dos reguladores, mais de 85% dos indivíduos têm por hábito consumir frutas, verduras folhosas e legumes, mas, ao se avaliar o consumo diário, verificase que a prática é maior na região de melhor nível socioeconômico. As informações dietéticas mostram que os idosos analisados apresentam o mesmo padrão alimentar de outros grupos populacionais no tocante aos alimentos energéticos, porém, diferem quanto aos protéicos e reguladores.Descritores: Nutrição. IntroduçãoNo Brasil, em geral, e, mais marcadamente, em algumas regiões do país, o processo de envelhecimento que está ocorrendo é comparável ao dos países desenvolvidos sendo que, nestes, a fase é de estabilização do crescimento da população de idosos, enquanto aqui esta fase está em franca escalada (Ramos e col. 15 , 1987; Veras e col. 19 , 1987). Sabe-se que o processo de envelhecimento coincide com uma redução progressiva dos tecidos ativos do organismo, perda da sua capacidade funcional e modificação das funções metabólicas (Burtis e col. ou indiretamente com a quantidade e qualidade de alimentos consumidos. RESUMOObjetivo: Avaliar a orientação nutricional dentro de um conjunto de intervenções multidisciplinares. Sujeitos e métodos: Quarenta e sete pacientes diabéticos tipo 2, hiperglicêmicos, tratados de forma convencional (n = 19) ou intensiva (n = 28) e avaliados pela glicemia média semanal (GMS) no início e após 6 semanas. Resultados: GI reduziu o consumo de calorias (p = 0,001), carboidratos (p = 0,004), lipídios (p = 0,001) e aumentou o de fibras, enquanto o GC reduziu o consumo de fibras (p = 0,018). Controle glicêmico (GMS ≤ 150 mg/dL) ocorreu em 75% do GI e, em 31,6% do GC (p = 0,003), houve correlação negativa entre as variações do consumo de fibras e a GMS (r = -0,309; p = 0,035). Os resultados mantiveram-se por 12 semanas. Conclusão: A intervenção educacional intensiva de curto prazo mostrou-se mais eficaz que o tratamento convencional para a obtenção do controle glicêmico. Nossos resultados ainda indicam que um consumo mais adequado de fibras na alimentação contribui para a obtenção de um melhor controle da glicemia. Subjects and methods: Forty-seven patients with type 2 diabetes and hyperglycemia (A1C ≥ 8%), treated conventionally (n = 19, GC) or intensively in six weekly visits (n = 28, GI) were analyzed. We evaluated mean weekly blood glucose (MWG) at baseline and after 6 weeks in both groups. Results: GI reduced caloric (p = 0.001), carbohydrate (p = 0.004), and fat (p = 0.001) intake, and increased fiber consumption, while GC reduced fiber intake (p = 0.018). Glycemic control (MWG ≤ 150 mg/dL) occurred in 75% of GI patients and in 31.6% of CG patients (p = 0.003), with negative correlation between changes in fiber intake and MWG values (r = -0.309; P = 0.035). Results were maintained after 12 weeks. Conclusion: Educational short-term intensive intervention was more effective than conventional treatment to achieve glycemic control.
