The matrix effect in polycondensation was first discovered and thoroughly studied using the example of acceptor-catalytic polyesterification. This effect is due to the influence of a filler on the reaction kinetics and the structure and properties of macromolecules produced. The filler (graphite or molybdenum disulfide), introduced into the reaction system during the growth of macromolecules, has been found to exhibit the properties of a heterogeneous matrix. Namely, it determines the kinetic parameters of the reaction, the molecular weight of the polymers, and the microstructure of the copolymers, as well as properties of composites produced thereby. A mechanism was proposed to explain the matrix effect observed.
Актуальность. Дисплазия тазобедренных суставов и врожденный вывих бедра -часто встречающаяся ано-малия развития у новорожденных. По-прежнему не редки случаи позднего выявления заболевания, неудачи в лечении и развитие осложнений, что не находит должного отражения в современной литературе. Цель исследования -выявить наиболее распространенные ошибки на этапах диагностики и начала лечения детей первого года жизни с врожденным вывихом бедра. Материалы и методы. Работа основана на ретроспективном анализе данных анамнеза: истории диагности-ки и предшествующего лечения, а также клинического, сонографического, лучевого, магнитно-резонансно-го методов обследования 250 пациентов от 5 мес. до 2 лет, поступивших в специализированное отделение патологии тазобедренного сустава ФГБУ «НИДОИ им. Г.И. Турнера» с диагнозом «врожденный вывих бедра» в 2005-2015 гг. Результаты. Наиболее распространенными причинами ошибок на этапе диагностики врожденного вывиха бедра у детей являются отсутствие осмотра новорожденного ортопедом в роддоме и проведения сонографи-ческого скрининга, а также погрешности в его выполнении. При начале лечения отмечалось использование конструкций, не обеспечивающих оптимальное положение для вправления головки бедра, а также поздняя установка показаний к первичному хирургическому лечению в случаях невправимых вывихов бедра.Ключевые слова: врожденный вывих бедра, дисплазия тазобедренных суставов, диагностика, лечение.
Background. The formation of multiplanar deformities of the proximal femur, in most cases combined with hypertrophy of the greater trochanter (relative overgrowth of the greater trochanter (ROGT)) and its high position relative to the femoral head, up to the development of pelvic and pelvic spine syndrome (trochanteric-pelvic impingement), has been considered one of the most common problems in the treatment of children with hip joint pathology of various etiologies. Aim. The aim of this study was to determine the causes of and characterize the X-ray anatomical changes in children with ROGT. Materials and methods. This study is based on an analysis of the survey results of 350 children 3 to 17 years old with an emerging high position of the greater trochanter due to various diseases of the hip joint. Details of the radiological indicators characterizing the change in the growth of the greater trochanter relative to the head and neck of the thigh were examined in 68 of these children (112 joints). Results. Most often, hypertrophy of the greater trochanter was observed in children with the sequele of ischemic disorders that occurred during the conservative treatment of hip dysplasia and developmental hip dislocation, as well as due to previous hematogenous osteomyelitis. It was revealed that in the affected hip joints, there was a regular decrease in the articulo-trochanteric distance index; simultaneously, TTD values, which characterize the isolated growth of the greater trochanter, were almost the same in normal and pathological conditions (p 0.05). Conclusion. Damage to the growth plates of the pineal gland and neck of the femur of various etiologies was the reason for ROGT formation. The X-ray anatomical changes include progressive shortening of the femoral neck. Moderately pronounced in preschool-age children, they progress with the childs growth and become the cause of chronic trauma injuries of the components of the hip joint.
Введение. Все чаще в различных отраслях хирургии при планировании и выполнении оперативных вмеша-тельств применяются 3D-моделирование и прототипирование. В ортопедии данная технология впервые по-явилась в 1990 г. при выполнении операции на коленном суставе. С тех пор началась разработка протоколов создания и применения индивидуальных шаблонов для навигации при хирургических вмешательствах на раз-личных костях скелета. Цель исследования -разработка нового трехмерного метода планирования и выполнения корригирующей остеотомии бедренной кости с использованием индивидуального шаблона, а также выявление преимуществ предложенного метода по сравнению со стандартной методикой планирования и выполнения оперативного вмешательства. Материалы и методы. Представлен новый способ планирования и выполнения корригирующей остеотомии бедра у детей с различной патологией тазобедренного сустава. Проведен анализ результатов планирования и выполнения корригирующей остеотомии бедра у 27 пациентов в возрасте от 5 до 18 лет (32 тазобедренных сустава) с врожденной и приобретенной деформацией бедренной кости. Заключение. Использование компьютерного 3D-моделирования при планировании и выполнении корригиру-ющих вмешательств на бедренной кости улучшает результаты лечения за счет практически идеальной точно-сти выполнения; минимизации возможных субъективных ошибок; сокращения времени операции; уменьше-ния лучевой нагрузки на пациента.Ключевые слова: дети, корригирующая остеотомия бедра, 3D-моделирование, индивидуальный шаблон. Introduction. Three-dimensional (3D) modeling and prototyping are increasingly being used in various branches of surgery for planning and performing surgical interventions. In orthopedics, this technology was first used in 1990 for performing knee-joint surgery. This was followed by the development of protocols for creating and applying individual patterns for navigation in the surgical interventions for various bones. Aim. The study aimed to develop a new 3D method for planning and performing corrective osteotomy of the femoral bone using an individual pattern and to identify the advantages of the proposed method in comparison with the standard method of planning and performing surgical intervention.
