El ojo es uno de los órganos en el cuerpo humano más pequeño y complejo, y como muchos otros órganos del cuerpo, presenta cambios en el embarazo, también puede verse afectado durante este periodo. Sin embargo, en la mayoría de las veces hay cambios fisiológicos, y menos común, patológicos. Es sabido que los cambios oculares durante el embarazo se ven limitados prácticamente al conocimiento de un cirujano oftalmólogo, por lo que es importante mencionar las principales características de estos, y destacar los cambios que pudiesen ser patológicos, de manera que se pueda contar con una guía y conocimiento claro, desde el primer nivel, hasta el médico gineco-obstetra. En este artículo, se revisará la literatura y resumen de los últimos cinco años sobre los cambios oculares durante el embarazo.
El pterigión es una de las patologías oculares más comunes que se presentan en la consulta diaria de oftalmología. Es por la frecuente presencia de esta patología, en la práctica clínica y en los quirófanos, que se debe tener actualización y proveer las soluciones más eficientes y seguras para el pterigión. El interés de este artículo es realizar una revisión de esta patología, y hacer un enfoque en aspectos novedosos que se hayan desarrollado, tanto para el manejo quirúrgico como para otros aspectos asociados con el pterigión. La diabetes gestacional es una enfermedad crónica, no transmisible, que se presenta por primera vez durante el embarazo. Es una afectación que, de no captarla tempranamente, puede tener graves efectos tanto en el feto como en la madre. Existen múltiples factores predisponentes a esta enfermedad, sobresaliendo la obesidad y la resistencia a la insulina, entre otros. Sin embargo, a lo largo de los años, se han debatido distintos métodos de detección temprana sobre la diabetes gestacional, todos con el fin de evitar complicaciones materno-fetales, las cuales han tenido y continúan teniendo un gran impacto en la salud mundial. Un alto porcentaje de mujeres con diabetes gestacional puede ser detectado tempranamente dependiendo de los criterios utilizados y su estrategia. No obstante, no existe suficiente evidencia que proporcione criterios diagnósticos tempranos de la diabetes gestacional. En este artículo se procederá a realizar una revisión bibliográfica, sobre los métodos de uno o dos pasos como tamizaje de la diabetes gestacional.
Casi la mitad de los nacimientos pretérminos son causados por procesos inflamatorios, que terminan resultando en una labor de parto prematuro. Estos niños nacidos en un periodo temprano, los hace altamente vulnerables al no tener, en la mayoría de las veces, la madurez en su organismo, destacándose de esta manera su sistema respiratorio. Muchos de ellos se verán en la necesidad de oxígeno suplementario, tratamientos e invasiones, así como largas estancias hospitalarias que, a la larga, llevan a un mayor riesgo de contagio de microorganismos, que terminan complicando su evolución. La mortalidad de estos niños se ve especialmente elevada, sobre todo en los nacidos previo a las 28 semanas de gestación. El uso de corticoesteroides prenatales ha demostrado la disminución de la morbilidad respiratoria en esta población, reduciendo sobre todo la aparición de taquipnea transitoria en el neonato y el síndrome de distrés respiratorio. El uso de corticoesteroides es de las terapias más utilizadas en el manejo del parto pretérmino, y de las más importante como parte de una buena evolución en el neonato. En este artículo, se procederá a realizar una revisión bibliográfica de los últimos estudios y consensos sobre el uso de corticoesteroides prenatales, así como sus beneficios y consecuencias.
