La diversidad y el aumento de instituciones de educación terciaria, junto con el incremento de oferta de carreras, posibilita el acceso a la educación superior a segmentos de la población hasta ahora excluidos, esto se traduce en un nuevo perfil de estudiante que se inserta en la educación terciaria. Asimismo, la reforma en salud, los cambios demográficos, epidemiológicos, sociales, culturales, económicos, el avance sostenido de enfermería como profesión y disciplina, exigen un replanteamiento en la formación y proponer un perfil de egreso acorde a las realidades y a las proyecciones enunciadas. Conscientes de esta situación se diseñó un currículo basado en competencias, centrado en el estudiante, que posibilite movilidad estudiantil. Para realizar el diseño precedentemente indicado se estableció un convenio de colaboración entre las cuatro instituciones de la red, se contrató a expertos que orientaron el diseño curricular, acorde a las tendencias en la disciplina de enfermería y educación. Se visitaron diferentes países para recabar experiencia; se analizaron documentos, se realizaron talleres con participación de diversos estamentos, donde se consideraron las competencias que se requieren en el ámbito laboral y concordar el perfil de egreso. Como productos se elaboró un marco contextual, con una amplia mirada, donde se relacionan los principales conceptos que iluminan la propuesta curricular, se definió un perfil y las competencias de egreso, se elaboró el plan de estudio asignando créditos a las asignaturas acorde al sistema de crédito transferible propuesto por los vicerrectores de las universidades adscritas al Consejo de Rectores de Chile.
Objective: To analyze, from an organizational perspective, the problem of nurse burnout in highly complex public hospitals in Chile. Method: Multicenter, observational, cross-sectional study. According to established inclusion criteria, the universe of hospitals and nurses was the object of work. Data collection was carried out through a nurse survey. Work environment was measured with the instrument Practice Environment Scale of the Nursing Work Index, staffing through the nurses’ report on patient load, and burnout with the Maslach Burnout Inventory emotional exhaustion subscale. Results: Thirty-four hospitals (92%) and 1,395 nurses (75.3%) participated in the study. The prevalence of burnout was 34.7%, being higher in Santiago than in other regions of the country (p = 0.001). The logistic regression analyses showed a significant association between work environment and burnout (OR 0.57, 95% CI 0.41–0.79, p = 0.001). No association was established between staffing, skill mix and burnout. Conclusion: A high percentage of nurses in Chile has burnout, which is significantly associated with the quality of the environment. The implementation of organizational strategies to improve work environments could reduce burnout and improve the quality of care.
La formación de profesionales siempre ha constituido un gran desafío. Sin embargo, hoy esto se agudiza, debido a las nuevas exigencias que surgen del avance de la globalización y de la sociedad del conocimiento; la fuerte expansión de la cobertura de la educación universitaria, que ha favorecido el ingreso de estudiantes con un nuevo perfil. Dicha situación, reclama nuevas aproximaciones didácticas, metodológicas y evaluativas de parte de los académicos a objeto de satisfacer las necesidades de formación del contingente de estudiantes que ingresa, el cual es cada más heterogéneo -producto del aumento de cobertura- y de una educación superior cada vez más masiva (1); este escenario exige a las instituciones generar un conjunto de acciones formativas que provean a los estudiantes oportunidades para el desarrollo de habilidades que les permitan enfrentar tanto la educación superior como las demandas laborales. Esto conlleva a que las instituciones de educación se vean enfrentadas a definir y actualizar los programas de formación profesional en una perfecta sincronía entre el avance disciplinar, las orientaciones ministeriales, las necesidades y demandas de la formación y laborales, impulsándolas a que remiren sus procesos formativos, replanteen los currículos, se interroguen respecto de cómo aprenden los estudiantes, se incorporen nuevas metodologías y recursos de enseñanza, y se materialicen sistemas de evaluación que permitan evidenciar los aprendizajes de los estudiantes. La necesidad de formar profesionales altamente calificados, el desarrollo de la ciencia y la tecnología, hace necesario una formación integrada, situada en itinerarios formativos con altos estándares, que habiliten a los egresados no sólo en el ámbito de la profesión, sino también aseguren el desarrollo ético, valórico, junto a una identidad profesional.
La muestra estuvo constituida por setenta y cinco docentes que representaban distintas áreas disciplinares y académicas. Las respuestas evidencian que, en las cuatro facultades, la evaluación está direccionada de acuerdo a lo establecido por los programas de las asignaturas, en cuanto a los objetivos a lograr y la naturaleza de los contenidos. Se evalúan principalmente conocimientos, aplicando pruebas de desarrollo, disertaciones y trabajos grupales. Respecto a los dominios taxonómicos, se observa un predominio en el nivel de comprensión, seguido de aplicación, dependiendo de la facultad y de las carreras. Si bien hay distintas concepciones por parte de los docentes sobre la evaluación, ésta se concibe como un proceso en el acto educativo que se inicia, en algunos casos, con el diagnóstico de los saberes que los estudiantes traen y concluye con la evaluación final.
