A violência sexual é definida como qualquer ato sexual, tentativa de obter um ato sexual, comentários ou investidas sexuais indesejadas, ou atos direcionados ao tráfico sexual. Embora seja uma realidade na população geral, as mulheres são as que mais sofrem esse tipo de agressão, nesse contexto a OMS apontou que 35% dessas relataram ter sofrido algum tipo abuso durante a vida. As informações sobre violência e atendimento prestado ocorre por meio da notificação ao Sistema de Informação de Agravos de Notificação (SINAN), que elabora e implementa políticas públicas de enfrentamento à violência. A atenção básica (AB), como porta de entrada, tem um papel importante no cuidado à mulher agredida sexualmente, minimizando os danos sofridos. O objetivo deste estudo é revisar através da literatura científica a importância da abordagem integral da atenção básica às vítimas de violência sexual. A AB em um caso de violência sexual a mulher deve não só oferecer apoio emocional, mas também evitar futuros danos a essa vítima, desde o acolhimento, como também oferecendo testes de rastreio a infecções sexualmente transmissíveis, contracepção de emergência e as orientações para o aborto legal se ocorrer uma gestação. O trabalho de amparo, oferecido pela AB às mulheres lesadas pela nítida expressão de desigualdade de gênero, tem inegável indispensabilidade no país, onde os números de casos de violência, notificados, são alarmantes. O cuidado íntegro àquelas que chegam a procura de ajuda é de suma relevância para congregar ao enfrentamento e acrescer às políticas públicas voltadas para casos de agressão em suas mais variadas formas.
O diabetes mellitus tipo 1 é a segunda doença crônica e distúrbio endócrino-metabólico mais frequente na infância. Tem sido cada vez mais diagnosticado nas crianças, devido ao amplo espectro de rastreamento. Estima-se uma prevalência entre 1,1 a 1,4/ 1000 crianças menores de 15 anos. Tal situação pode afetar a qualidade de vida das crianças, familiares. Os fatores ambientais podem influenciar nesse fator. A idade, sexo, tipo de seguro a saúde têm associação estatisticamente significativa com a melhor qualidade de vida. A educação materna, estado civil materno, o regime de insulina e realização de controle glicêmico modificam a qualidade de vida materno- infantil. A idade média de início da doença é de 8,8 anos, estreando com cetoacidose diabética. A hemoglobina glicosilada (HbA1c) é o outro ponto de alteração. Altos níveis são obtidos tanto na adesão quanto na qualidade de vida. A qualidade de vida está associada ao uso de um sistema de monitoramento contínuo de glicose, para se evitar hipoglicemias menos graves e complicações renais. Diante de tal situação, objetiva-se a realizar uma revisão de literatura sobre o tema para avaliar o impacto da DM1 e suas repercussões na vida da criança. Assim, percebe-se que tem impactado de forma direta na vida tanto da família, quanto da criança, principalmente na aceitação do novo estilo de vida. Logo, estudos são fundamentais para análise dos benefícios de se melhorar a qualidade de vida com apoio da equipe multiprofissional.
A insuficiência cardíaca (IC) é uma complicação que pode estar presentes nos pacientes acometidos por diabeter mellitus tipo 2 associados a riscos aumentados de morbidade e mortalidade. A IC é comum e associada a mau prognóstico, apesar dos avanços no tratamento. Os principais processos biológicos que caracterizam a insuficiência cardíaca com fração de ejeção preservada são inflamação sistêmica, acúmulo de tecido adiposo epicárdico, rarefação da microcirculação coronariana, fibrose miocárdica e rigidez vascular. O comprometimento resultante da distensibilidade ventricular esquerda e aórtica (especialmente acompanhada de função glomerular prejudicada e retenção de sódio), causa aumento nas pressões de enchimento cardíaco e dispneia de esforço, apesar da preservação relativa da fração de ejeção do ventrículo esquerdo. Os inibidores do cotransportador de sódio-glicose 2 (SGLT2) é uma das classes de terapias de redução de glicose que tem demonstrado benefícios de saúde multissistêmicos. Os três inibidores que são utilizados são: empaglifozina, canaglifozina e dapaglifozina. A empaglifozina reduziu o risco de falência da bomba e morte súbita, os dois modos mais comuns em pacientes com insuficiencia cardíaca. Independentemente de suas ações sobre a glicemia, os inibidores de SGLT2 exercem ampla gama de efeitos biológicos incluindo ações para inibir a inflamação cardíaca e a fibrose, antagonizar a retenção de sódio e melhorar a função glomerular que podem melhorar os distúrbios fisiopatológicos da insuficiência cardíaca.
