Objetivo: Avaliar o comportamento da COVID-19 em municípios de pequeno porte. Métodos: Estudo transversal, retrospectivo, com dados de fichas de notificação do sistema Notifica COVID-19 da 14° Regional de Saúde do estado do Paraná, entre março de 2020 e maio de 2021, após os dados foram analisados em planilhas eletrônicas, com cálculo de prevalência e incidência. O projeto foi aprovado pelo Comitê de Ética em Pesquisa. Resultados: Das 86.848 fichas analisadas, 21.061 eram pacientes com diagnóstico positivo para COVID-19. Destes, 16.114 eram pacientes recuperados, 4.504 com doença ativa e 443 óbitos. Quanto à incidência de casos, maiores em 2021, nos meses de fevereiro, março e maio. Os óbitos ocorreram em maiores de 20 anos, 42% tinham comorbidades, destacando-se: Hipertensão arterial, Diabetes Mellitus e Obesidade. Conclusões: A regional analisada, mesmo não sendo uma das mais populosas, apresentou prevalência, incidência e mortalidade e o estudo contribuiu para alertar as autoridades sanitárias no controle de transmissão e intensificação das restrições.
