Este estudo objetivou compreender os aspectos emocionais de crianças e adolescentes com diabetes tipo 1 (DM1) e seus familiares sob um olhar psicanalítico. Trata-se de um estudo qualitativo realizado por meio de entrevistas semiestruturadas com núcleos familiares de crianças e adolescentes com DM1. Os dados foram analisados de acordo com a análise de conteúdo, elegendo-se três categorias: diagnóstico, atendimento e tratamento. As crianças e adolescentes demonstraram sofrimento psíquico relacionado ao diagnóstico e internação hospitalar, com vivências de luto, sentimentos de desintegração e desproteção. Também denotaram sofrimento associado à restrição alimentar e aplicação da insulina. Continuidade do cuidado e disponibilidade dos membros da equipe interprofissional de saúde refletiram positivamente sobre o sentimento de segurança tanto dos familiares quanto das crianças e dos adolescentes. Na perspectiva da psicanálise, os aspectos emocionais observados nos núcleos familiares de crianças e adolescentes com DM1 afetaram sua condição de saúde, interferindo na aceitação, adesão e controle da doença.
A adolescência, enquanto estágio de desenvolvimento humano tem sido conceituada como transição entre a infância e a fase adulta, sendo definida pela OMS como um período compreendido entre 10 e 19 anos (WHO, 2016). A saúde do adolescente constitui um desafio para os profissionais da saúde visto que, apesar do crescente interesse nas últimas décadas por esta população, a sociedade comumente exige do adolescente o cumprimento dos seus deveres como cidadão, mas não tem dado a devida atenção quanto aos direitos e às necessidades deste ciclo de vida (RUZANY, 2008). Entendendo as especificidades da adolescência e tomando como base as Diretrizes Nacionais para a Atenção Integral à Saúde de Adolescentes e Jovens do Ministério da Saúde (BRASIL, 2010), este trabalho tem por objetivo relatar o projeto de pesquisa que teve início em 2017 e está em andamento em um bairro do município de Blumenau/SC. O estudo envolve a aplicação de questionário sociodemográfico com os pais/responsáveis e a realização de grupos focais com os adolescentes da escola do bairro e entrevistas com profissionais de saúde da Estratégia Saúde da Família da mesma localidade. A pesquisa foi aprovada pelo Comitê de Ética na Pesquisa em Seres Humanos com os números 2.558.376 e 2.216.628/2017. A finalidade é compreender as concepções de saúde expressas pelos adolescentes e pelos profissionais de saúde, ancorados em uma perspectiva holística e positiva da saúde (NORDENFELT, 2000), visando promover junto à comunidade reflexões e ações de saúde sobre a adolescência. Os resultados parciais indicam que os adolescentes conceituam saúde como qualidade de vida e bem-estar e que possuem informações sobre as condições para uma vida saudável, mas referem dificuldade para colocá-las em prática. Não ter saúde foi apresentado como processos físicos e mentais como doenças e histórias de perdas familiares que os impedem de realizar ações consideradas importantes para eles. Quanto aos profissionais de saúde, estes expressaram histórico de dificuldades em relação à sua própria adolescência e resistência no trabalho com o público deste ciclo de vida. Mesmo reconhecendo a importância do acolhimento ao adolescente e do trabalho diferenciado para este grupo, os profissionais declaram que ainda há escassa inserção do adolescente no planejamento de ações da atenção básica e que possuem dificuldade para atingir este público. Percebe-se que conhecer as condições sociodemográficas e as concepções de saúde dos adolescentes é essencial para o planejamento de ações de saúde que atendam as características daquela comunidade. Aliado a estas questões, a escuta dos profissionais e a análise de suas trajetórias de vida, pode possibilitar a construção de um ambiente sensível, acolhedor e promotor de saúde para os adolescentes. Outrossim, a perspectiva positiva de saúde pode contribuir para a construção de estratégias que estimulem o protagonismo juvenil e o bem-estar durante a adolescência, favorecendo uma vida adulta saudável.
