Introdução: A doença hepática gordurosa não alcoólica (DHGNA) é um grande desafio da prática médica atual, está associada com doenças metabólicas e cardiovasculares principalmente a doença arterial coronariana aterosclerótica. Sua prevalência estimada em 32,4%, vem aumentando progressivamente e apresenta variações geográficas, étnicas, entre gênero e idade. Objetivo: Estimar a prevalência da DHGNA em portadores de doença arterial coronariana aterosclerótica atendidos no ambulatório de cardiologia de hospital público da região sul do Brasil. Métodos: Estudo transversal, desenvolvido de janeiro a outubro de 2022, envolvendo 31 pacientes submetidos à cirurgia de revascularização miocárdica, intervenção coronariana percutânea ou com histórico de infarto agudo do miocárdio. A presença de esteatose hepática foi avaliada pela ultrassonografia abdominal e graduada em leve, moderada ou acentuada. Resultados: A prevalência geral foi de 70,97%, 58,06% (leve) e 12,9% (moderada). Entre gêneros 77,78% (masculino) e 61,53% (feminino), entre etnias de 75% (brancos), 100% (negros) e 40% (pardos), relacionada a escolaridade 73,08% (ensino infantil/ fundamental), 50% (médio) e 100% (superior). Em indivíduos com peso normal de 66,67%, com sobrepeso 85,71% e em obesos 57,14%. Em praticantes de atividade física < moderada de 72% e moderada de 80%. Nos portadores de síndrome metabólica de 81,82%, 54,55% (leve) e 27,27% (moderado), em diabéticos de 70%, 50% (leve) e 20% (moderada). Conclusão: A DHGNA foi altamente prevalente, a associação com diabetes mellitus não aumentou a prevalência, diferentemente da presença da síndrome metabólica e sobrepeso, embora não se possa avaliar a significância desses dados, devido à pequena amostra de pacientes.
As doenças cardiovasculares são as principais causas de morte em todo o mundo e a obesidade, bem como a diabetes mellitus tipo 2 emergem como epidemia global. A prevenção é a base para a abordagem das doenças cardiovasculares para se reverter esse panorama sombrio e a conscientização por meio da informação pode aumentar a adesão da população em geral às mudanças nos hábitos de vida que são fundamentais nesse contexto. As informações obtidas em um atendimento básico como anamnese compacta e objetiva, aferição da pressão arterial e avaliação de glicemia capilar, como parâmetro para orientações individualizadas, associados a palestras educativas com linguagem acessível, podem ajudar a romper a barreira entre o conhecimento científico e a população. O envolvimento pleno do acadêmico da área de saúde nesse processo de transmissão de conhecimento, sob a coordenação de um professor e pesquisador, permite a conexão plena entre a pesquisa, o ensino e a extensão. Sob esses fundamentos, foi criado o projeto de extensão sob o título: prevenção primária e secundária de doenças cardiovasculares e diabetes tipo 2 como o objetivo de levar o conhecimento científico com linguagem apropriada para a população, tendo como um dos principais veículos o acadêmico da área de saúde.
Introduction: The therapeutic decision must be an active process between the health team and the patient. The benefits of treatment adherence are widely known, however, adherence is a challenge that has been described since 1970. Such a scenario reflects the increase in morbidity and mortality from cardiac conditions. The present study is based on actions coordinated by a multidisciplinary team, consisting of members of the pharmacy, social assistance, and cardiology services, aiming to identify and resolve the factors that may influence adherence to outpatient cardiac medical treatment and to evaluate the impact of these actions in this process. Methods: Longitudinal study that included 15 patients treated at the cardiology outpatient clinic of the Hospital Regional dos Campos Gerais, carried out from May to December 2022. At first, the patients were seen in the conventional cardiology consultation and, later, they were directed to the multidisciplinary consultation. The study evaluated sociodemographic and clinical variables and analyzed lifestyle, quality of life, and adherence to therapy using validated questionnaires. After a period, which varied from 5 to 7 months, the patients were reassessed and submitted again to the questionnaires. Results: After the multidisciplinary intervention, there was an improvement, mainly, in the parameters of adherence to pharmacological therapy, a reduction in the number of sedentary individuals, an increase in quality of life, and less nicotine dependence among smokers. There was no change in the profiles related to smoking and alcoholism and little change associated with changing the therapeutic regimen (number of medications used daily). Conclusion: The multidisciplinary approach in the cardiological context promotes improvement in patients' therapeutic adherence and lifestyle parameters, thus positively impacting the quality of life of patients with heart disease.
A insuficiência cardíaca (IC) permanece com prognóstico ruim e alta morbidade e mortalidade. A pandemia de SARS-CoV-2 pode ter influenciado esses parâmetros pela sobrecarga dos serviços de saúde, embora características regionais possam ter influenciado. OBJETIVO: Avaliar o impacto da pandemia de SARS-CoV-2 na morbimortalidade por IC no Nordeste do Brasil através da análise de dados do DATA-SUS. MÉTODO: Foram avaliados o tempo médio de internação, o número de internações e óbitos e a taxa de mortalidade de 02/2017 a 11/2022, subdividido em pré-pandemia (02/2017 a 02/2020), pandemia (03/2020 a 06/2021), pico (12/2020 a 05/2021), primeiro e segundo trimestre do pico (12 /2020 a 02/2021 e 03/2021 a 05/2021), vacinação em massa (06/2021 a 12/2021) e ano de 2022 (01/2022 a 11/20222022). As subdivisões do período total avaliado foram comparadas com as demais. Os resultados foram expressos em porcentagem. RESULTADOS: Na pré-pandemia houve a maior média mensal de internamentos, com redução de 29% na pandemia e de 12% no período de vacinação em massa. Houve redução na média mensal de óbitos no período de pandemia quando comparado à pré-pandemia, sendo essa média semelhante nos períodos de vacinação em massa e de pré-pandêmia. No entanto, houve aumento significativo da taxa de mortalidade no período de pandemia (12,45%) quando comparado ao período de pré-pandemia (10,9%), sendo o maior percentual observado no período de vacinação em massa (12,79%). CONCLUSÕES: Houve consequências relevantes da pandemia de SARS-CoV-2 na morbimortalidade por IC na região Nordeste do Brasil, com menor número de internações e aumento da mortalidade. Durante o período de vacinação, o aumento das internações e a maior taxa de mortalidade podem refletir tanto a dificuldade de acompanhamento da doença em períodos anteriores quanto o maior acesso aos serviços de saúde.
Heart failure (HF) remains with a poor prognosis and high morbidity and mortality. The SARS-CoV-2 pandemic may have influenced these parameters due to the overload of health services, although regional characteristics may have had an influence. OBJECTIVE: To assess the impact of the SARS-CoV-2 pandemic on HF morbidity and mortality in Northeast Brazil through data analysis from DATA-SUS. METHOD: The average length of stay, the number of hospitalizations and deaths, and the mortality rate from 02/2017 to 11/2022 were evaluated, subdivided into pre-pandemic (02/2017 to 02/2020), pandemic (03/2020 to 06/2021), peak (12/2020 to 05/2021), the first and second trimester of the peak (12/2020 to 02/2021 and 03/2021 to 05/2021), mass vaccination (06/2021 to 12/2021) and year 2022 (01/2022 to 11/20222022). The subdivisions of the total evaluated period were compared with the others. The results were expressed in percentages. RESULTS: In the pre-pandemic, there was the highest monthly average of hospitalizations, with a reduction of 29% in the pandemic and 12% in the period of mass vaccination. There was a reduction in the monthly average of deaths in the pandemic period compared to the pre-pandemic period, with this average being similar in the mass vaccination and pre-pandemic periods. However, there was a significant increase in the mortality rate in the pandemic period (12.45%) when compared to the pre-pandemic period (10.9%), with the highest percentage observed in the mass vaccination period (12.79%). %). CONCLUSIONS: There were relevant consequences of the SARS-CoV-2 pandemic on HF morbidity and mortality in the Northeast region of Brazil, with a lower number of hospitalizations and increased mortality. During the vaccination period, the increase in hospitalizations and the higher mortality rate may reflect the difficulty in monitoring the disease in previous periods and greater access to health services.
