BackgroundUnderserved women (rural, minority or poor) are disproportionally diagnosed with late-stage cervical cancer, indicative of inadequate access to, and use of, preventative healthcare. The Institute of Medicine (IOM) has proposed that nurse practitioners (NP) can address provider shortages among underserved populations, but to reduce shortages, scope-of-practice laws that restrict the delivery of care, must be revised. We examined the IOM recommendation of NP expanded scope-of-practice laws on reducing the disparity of underserved women diagnosed with late-stage cervical cancer.MethodsWe examined the cohort of 10 673 women diagnosed with cervical cancer between 2010 and 2014 and reported to the Surveillance, Epidemiology and End Results cancer registry. We linked state-level laws regarding NP scope-of-practice to patients with cancer by their state of residence, diagnosis date and law enactment date. Hierarchical regression was used to explore NP full scope-of-practice law’s impact on late-stage cancer diagnoses considering the moderating effect of women living in medically underserved areas. We adjusted for known confounders available in this population-based data set.ResultsMedically underserved women living in states with laws that restrict NP full scope-of-practice are twofold more likely to be diagnosed with late-stage cancer; adjusted OR and 95% CI (OR 2.08, 95% CI 1.4 to 3.1). These disparities were not observed among underserved women living in areas with NP full scope-of-practice laws (OR 0.95, 95% CI 0.7 to 1.3).ConclusionsNP full scope-of-practice laws could provide a pragmatic and cost-effective solution to healthcare provider shortages associated with late stage of cervical cancer diagnoses among underserved women.
BACKGROUND AND OBJECTIVES: The difficulty of the nursing staff in assessing pain, the low adherence to the registry of pain and the quality reported in other studies brings the need to assess the related factors and one of them can be the lack of knowledge. The objective of this study was to evaluate the knowledge of nursing students about pain assessment. METHODS: A cross-sectional, descriptive and quantitative study, conducted in a Higher Education School in Aracaju, Brazil. The sample was composed of 169 nursing students over the last period of graduation. The data were obtained through a form that contained sociodemographic information, questions about pain assessment, and knowledge sources used. RESULTS: The scales to assess pain in children and in adults are known by 70 and 66% of the students, respectively. They reported that pain assessment and the incorrect pain treatment may impair the clinical picture and aggravate the disease, both in the child (17.2%) and the adult (15.4%). The rate of correct answers about pain assessment ranged from 26.6 to 87%. Most of them use articles to gain knowledge about pain (56.2%). The fact of having already used a scale to assess the patient's pain (p=0.045) showed significant differences in the average of hits. CONCLUSION: Nursing graduates are aware of the existence of scales to assess pain, but they do not have the ability to perform them. The prior use of scales promotes learning. The lack of knowledge about pain impacts on the patient's health.
Analisar a positividade e os fatores de risco associados à infecção da esquistossomose mansoni e das enteroparasitoses em área endêmica, Sergipe, Brasil. Trata-se de um estudo transversal. O estudo envolveu 160 moradores do povoado Siebra, situado na zona rural do município de Maruim, Sergipe. As positividades para Schistosoma mansoni e/ou enteroparasitos foram identificadas por meio do método Kato-Katz e do método de Ritchie, respectivamente, e correlacionados com as condições socioambientais da região. Em relação a positividade para esquistossomose foi de 49,37%; a infecção prevaleceu no sexo masculino (52,70%); em idosos (77,77%). Os maiores riscos de se infectar com Schistosoma mansoni foi em indivíduos do sexo masculino, em idade (até 9 a >40 anos), que residem em área rural, recebem ajuda governamental, que tem constante contato com águas naturais e que recebem informações sobre as parasitoses. Quanto às amostras analisadas para as enteroparasitoses, os protozoários Endolimax nana e Giardia lamblia (97,52% e 78,75%) foram os mais prevalentes; o Endolimax nana esteve mais prevalente em todas as faixas; a infecção prevaleceu no sexo feminino (42,52%) para Trichuris trichiura, 9,19% para Enterobius vermiculares, 67,81% para Entamoeba histolytica, 85,05% para Giardia lamblia, 88,50% para Entamoeba coli e 86,20% para Blastocytis hominis). O estudo revelou que Maruim é endêmico para a esquistossomose e para as enteroparasitoses e que as condições socioambientais e sanitárias contribuem com as altas taxas de positividade. Os resultados encontrados mostram a necessidade do fortalecimento de programas de saúde com o objetivo de reduzir a transmissão e sensibilizar a população em relação a essas doenças.
O controle da dor no recém-nascido é fundamental para uma melhor assistência, portanto sua mensuração é indispensável. Objetivo: Descrever através da literatura os principais métodos de avaliação da dor e as intervenções não farmacológicas utilizadas pela equipe de enfermagem no recém-nascido. Foi realizada uma pesquisa bibliográfica de artigos nacionais e oriundos da base de dados eletrônicas BVS durante o período de 2017 a março de 2018. Foram analisados 13 artigos ao todo. No presente estudo a equipe de enfermagem relatou em alguns artigos que usam os métodos não farmacológicos e a exemplo tem-se sucção não nutritiva com glicose ou não, mudança de decúbito, enrolamento, etc. Quanto a avaliação a dor ela foi avaliada de forma empírica sem a utilização de escalas de avaliação da dor padronizada em boa parte dos estudos. Poucos meios validados são utilizados para avaliar a dor neonatal.
O estudo visa comparar a eficácia e a sensibilidade de três técnicas parasitológicas no diagnóstico das helmintíases em uma área endêmica no nordeste do Brasil. Trata-se de um estudo epidemiológico, descritivo e comparativo, o mesmo foi realizado no município de Maruim, que fica aproximadamente a 30 km de da capital do estado de Sergipe, a cidade de Aracaju. Foram distribuídos kits e coletadas 3 amostras diárias entre os dias 17 a 20 de setembro de 2018, posteriormente as amostras foram transportadas até o laboratório de Entomologia e Parasitologia Tropical da Universidade Federal de Sergipe para realizar análise macroscópica, aplicando o método de Kato-Katz, Hoffman, Pons e Janer, e o método de Ritchie. Foi encontrado uma maior prevalência de casos positivos para Schistosoma mansoni (48,12%), ancilostomídeos (4,38%) e Hymenolepis nana (2,50%) quando analisado sob a técnica de Kato -Katz, os helmintos Enterobius vermicularis (5,62%), Trichuris trichiura (36,25%) e Strongyloides stercoralis (5,00%) mostrou-se mais encontrado quando utilizado a técnica de Ritchie. As técnicas se mostraram bastante eficientes na identificação dos parasitas, porém nem todos os parasitos foram identificados simultaneamente pelas técnicas. Com base nesta informação, conclui-se que uma técnica pode complementar a outra no diagnóstico parasitário aumentando assim sua sensibilidade.
