The safe disposal of human excreta is of paramount importance for the health and welfare of populations living in low income countries as well as the prevention of pollution to the surrounding environment. On-site sanitation (OSS) systems are the most numerous means of treating excreta in low income countries, these facilities aim at treating human waste at source and can provide a hygienic and affordable method of waste disposal. However, current OSS systems need improvement and require further research and development. Development of OSS facilities that treat excreta at, or close to, its source require knowledge of the waste stream entering the system. Data regarding the generation rate and the chemical and physical composition of fresh feces and urine was collected from the medical literature as well as the treatability sector. The data were summarized and statistical analysis was used to quantify the major factors that were a significant cause of variability. The impact of this data on biological processes, thermal processes, physical separators, and chemical processes was then assessed. Results showed that the median fecal wet mass production was 128 g/cap/day, with a median dry mass of 29 g/cap/day. Fecal output in healthy individuals was 1.20 defecations per 24 hr period and the main factor affecting fecal mass was the fiber intake of the population. Fecal wet mass values were increased by a factor of 2 in low income countries (high fiber intakes) in comparison to values found in high income countries (low fiber intakes). Feces had a median pH of 6.64 and were composed of 74.6% water. Bacterial biomass is the major component (25–54% of dry solids) of the organic fraction of the feces. Undigested carbohydrate, fiber, protein, and fat comprise the remainder and the amounts depend on diet and diarrhea prevalence in the population. The inorganic component of the feces is primarily undigested dietary elements that also depend on dietary supply. Median urine generation rates were 1.42 L/cap/day with a dry solids content of 59 g/cap/day. Variation in the volume and composition of urine is caused by differences in physical exertion, environmental conditions, as well as water, salt, and high protein intakes. Urine has a pH 6.2 and contains the largest fractions of nitrogen, phosphorus, and potassium released from the body. The urinary excretion of nitrogen was significant (10.98 g/cap/day) with urea the most predominant constituent making up over 50% of total organic solids. The dietary intake of food and fluid is the major cause of variation in both the fecal and urine composition and these variables should always be considered if the generation rate, physical, and chemical composition of feces and urine is to be accurately predicted.

In 2017, an estimated 1 in every 4 (23%) children aged < 5 years were stunted worldwide. With slow progress in stunting reduction in many regions and the realization that a large proportion of stunting is not due to insufficient diet or diarrhea alone, it remains that other factors must explain continued growth faltering. Environmental enteric dysfunction (EED), a subclinical state of intestinal inflammation, can occur in infants across the developing world and is proposed as an immediate causal factor connecting poor sanitation and stunting. A result of chronic pathogen exposure, EED presents multiple causal pathways, and as such the scope and sensitivity of traditional water, sanitation, and hygiene (WASH) interventions have possibly been unsubstantial. Although the definite pathogenesis of EED and the mechanism by which stunting occurs are yet to be defined, this paper reviews the existing literature surrounding the proposed pathology and transmission of EED in infants and considerations for nutrition and WASH interventions to improve linear growth worldwide.

Effective menstrual management is essential for the mental and physical well being of women. However, many women in low income countries lack access to the materials and facilities required. They are thus restricted in their activities whilst menstruating thus compromising their education, income, and domestic responsibilities. This study describes the menstrual management challenges faced by women in an emergency situation in Uganda. Fifty interviews and focus group discussions were conducted with women from villages, IDP camps, and schools so that the menstrual management of the host population could be compared with the IDPs. This study showed that in IDP camps there was a significant lack of materials including soap, underpants and absorbing cloth, and facilities like latrines and bathing shelters. As a consequence women in IDP camps suffer with poor health and diminished dignity. There is also a lack of education about menstruation and reproductive health and practices are strongly influenced by cultural taboos.

Global climate change is affecting Africa, as it is every other continent and region of the world. The absolute poverty of a large proportion of the continent's people renders them highly vulnerable to changes in climate. Mitigation of climate change is a global imperative. However, numerous other changes continue apace, notably population growth, natural resource degradation, and rural-urban migration. Probably 50% or more of the continent's population rely on groundwater. This paper explores the relative impacts of changes in climate, demography and land use/cover on groundwater resources and demands. It concludes that the climate change impacts are likely to be significant, though uncertain in direction and magnitude, while the direct and indirect impacts of demographic change on both water resources and water demand are not only known with far greater certainty, but are also likely to be much larger. The combined effects of urban population growth, rising food demands and energy costs, and consequent demand for fresh water represent real cause for alarm, and these dwarf the likely impacts of climate change on groundwater resources, at least over the first half of the 21st century.Key words climate change; population; groundwater; recharge; Africa Changement climatique, tendances démographiques et eaux souterraines en AfriqueRésumé Le changement climatique global affecte l'Afrique, comme chaque autre continent et région du monde. Cependant de nombreux autres changements se développent rapidement, notamment la croissance démographique, la dégradation des ressources naturelles et l'exode rural. Probablement 50% ou plus de la population du continent comptent sur l'eau souterraine. Cet article explore les impacts relatifs des changements climatiques, démographiques et d'occupation du sol sur des ressources en eaux souterraines et les besoins associés. Il conclut que les impacts du changement climatique vont probablement être significatifs, quoiqu'incertains en direction et en ampleur, tandis que les impacts directs et indirects des changements démographiques sur les ressources et sur les demandes en eau sont non seulement connus avec une bien meilleure certitude, mais devraient également être bien plus importants. Les effets combinés de la croissance démographique urbaine, et de l'augmentation des besoins alimentaires et des coûts énergétiques, et par voie de conséquence de la demande en eau douce, représentent une véritable source d'inquiétude et éclipsent les impacts potentiels du changement climatique sur les ressources en eaux souterraines, au moins pendant la première moitié du 21ème siècle.

Menstrual hygiene management (MHM) has gained some attention and several literature reviews have been published. However, both original papers and reviews tend to focus on absorbent access and use and not on the disposal of menstrual waste. This review aims to fill a gap in the water, sanitation and hygiene (WASH) sector by bringing a focus specifically on menstrual hygiene safe disposal in low- and middle-income countries (LMIC). We reviewed published literature since 2002 on menstrual hygiene with a focus on menstrual waste management and menstrual absorbent disposal in LMIC. Database searches were conducted of both peer reviewed literature and grey literature, in addition to hand searching of references of relevant earlier literature reviews. In total 152 articles and reports were identified and 75 met the inclusion criteria and was included in the final review. Existing polices on MHM was also reviewed with a focus on India and South Africa. The review showed that disposal of menstrual waste is often neglected MHM and sanitation value chains, leading to improper disposal and negative impacts on users, the sanitation systems and the environment. Findings call for further research to gain better understandings of MHM waste streams, disposal behaviors, absorbent materials and waste management technologies to deliver health, safety, mobility and dignity for women and girls.
