O presente artigo apresenta uma revisão da Craniectomia (CT) e da Cirurgia Minimamente Invasiva (CMI), com o intuito de comparar a valência dessas técnicas no tratamento da Hipertensão Intracraniana (HIC), visto que a alta pressão intracraniana pode gerar déficits irreversíveis, como a herniação cerebral. Por intermédio da exposição de categorias e características, tanto da CT quanto da CMI, buscou-se avaliar a melhor conjuntura de aplicação de determinado método. O estudo esclarece a metodologia da CMI pela Remoção Neuroendoscópica e Aspiração Estereotáxica, assim como suas limitações e benefícios. De forma análoga, descreve-se o mecanismo de CT e discute-se o cenário mais propício para a sua utilização. Por fim, conclui-se, a partir dos dados, que a Cirurgia Minimamente Invasiva é favorável à redução da taxa de ressangramentos, do risco de edemas cerebrais e menor taxa de infecções, porém limitada na visualização necessária da hemostasia e no imediato alívio da pressão intracraniana, enquanto que a CT possibilita consequências indesejáveis e apresenta-se antagônico à homeostasia dos vasos.
Objetivo: Analisar as características da obesidade e o seu tratamento farmacológico e cirúrgico. Revisão bibliográfica: A obesidade é um dos principais problemas de saúde global da atualidade, sendo considerados obesos indivíduos com índice de massa corporal ≥ 30 kg/m 2. Os dados disponíveis sugerem que a carga global de obesidade mais do que triplicou desde 1975, correspondendo a 650 milhões de pessoas mundialmente. A obesidade está associada a várias comorbidades, como doenças cardiovasculares, síndrome metabólica, diabetes mellitus tipo 2(DM2), hiperlipidemia e hipertensão arterial (HAS). Considerações finais: A obesidade é um problema mundial de saúde pública que agravado pelas comorbidades que a acompanham. Seu tratamento é feito primeiramente com intervenções no estilo de vida baseadas em dieta, atividade física e intervenções comportamentais. Além disso, existem inúmeras opções farmacológicas que podem ser associadas dependendo das indicações do paciente. A cirurgia metabólica e bariátrica é recomendada para indivíduos com IMC > 34,9 kg/m 2 com comorbidades como HAS e DM2, sendo considerada como principal opção para IMC maior ou igual a 40 kg/m 2.
O tumor de Krukenberg (TK) é uma metástase neoplásica ovariana incomum, representa 1-2% dos cânceres incidentes no ovário, nos quais o estômago é a localização primária mais habitual. O objetivo do estudo foi relatar um caso de uma paciente de 42 anos, com TK metastático diagnosticado após laparotomia de emergência, relacionado a uma neoplasia gástrica sem diagnóstico prévio. A média de idade encontrada em pacientes com TK é 45 anos, sendo que a maioria dos casos relatados trata-se de TK bilateral. Os sintomas podem ser inespecíficos, com diagnósticos incidentais intraopertórios, ou relacionados ao envolvimento ovarianos, com dor e distensão abdominal e irregularidade menstrual. Comumente os pacientes podem ser assintomáticos, tendo manifestações clínicas somente muito tardiamente. A expressão de recptores de hormônios sexuais no TK ainda é indeterminada. Em relação ao prognóstico, a metastasectomia promove uma maior sobrevida aos pacientes, enquanto a cirurgia citorredutora revelou efeitos benéficos em relação a sobrevida, porém pode associar-se ao aumento da morbimortalidade e recorrências. Ainda não há consenso definido na literatura sobre o tratamento ideal, embora haja avanços nos estudos. O TK é um diagnóstico raro, acomente mais mulheres na pré-menopausa e tem um prognóstico ruim. A primeira recomendação para melhor sobrevida da paciente é a ressecção da lesão metastásica, seguida de terapia sistêmica paliativa.
Objetivo: Discutir sobre as terapias cirúrgicas utilizadas na abordagem da apendicite aguda em gestantes, seus riscos e segurança. Métodos: Foi realizado através de uma revisão integrativa de literatura, uma pesquisa nas bases de dados PubMed, BVS e Science Directs, com a utilização dos descritores “acute appendecitis”, “pregnancy” e “surgical treatment”. Foram aplicados critérios de inclusão e exclusão para seleção dos artigos, totalizando 23 artigos selecionados. Resultados: A seleção mostrou que a laparoscopia foi a cirurgia mais realizada e que esta apresenta diversos proveitos frente à laparotomia. A cirurgia laparoscópica além de apresentar benefícios no perioperatório, também apresenta no pós-operatório. É preciso, porém, ter maior cuidado durante a cirurgia visto o aumento uterino e a chance de perfurá-lo. Mesmo em menor número, a laparotomia apresenta certas considerações, porém, menos relevantes e, ainda, possui mais complicações. Considerações finais: Dessa forma, é observado que a cirurgia laparoscópica é o procedimento mais bem aceito e o mais realizado em gestantes com apendicite aguda, em razão de mostrar melhores desfechos
O câncer de mama masculino é uma entidade muito rara, que representa uma porcentagem muito pequena de todos os cânceres em homens. Homens não têm as mamas desenvolvidas, porém, assim como as mulheres, possuem tecido mamário, ainda que plano e pequeno, e podem desenvolver a doença, os subtipos mais comuns são o Carcinoma ductal in situ, carcinoma ductal invasivo e carcinoma lobular invasivo. Um dos primeiros sinais de alerta é o aparecimento de nódulo, aumento repentino das mamas e saída de secreção avermelhada. Este câncer é causado pela multiplicação desordenada de células anormais, formando um tumor que pode invadir localmente a parede torácica ou outros órgãos. Alguns fatores como envelhecimento, história familiar de câncer de mama, mutações genéticas, síndromes de hiperestrogenismo, alcoolismo e exposição à radiação; tem contribuição para o desenvolvimento da doença. Alguns cânceres apresentam desenvolvimento rápido, enquanto outros crescem lentamente. A maioria dos casos, quando tratados adequadamente e em tempo oportuno, apresentam bom prognóstico.
