RESUMO:Objetivo: avaliar a cultura de segurança organizacional entre os profissionais de um hospital de Ensino. Método: estudo tipo survey com 645 profissionais de um hospital de ensino no Estado do Paraná, de outubro de 2014 a julho de 2015, utilizando instrumento autoaplicável da Agency for Health care Research and Quality. Variáveis quantitativas foram representadas em percentuais; quando ≥75% das respostas foram positivas, cultura de segurança satisfatória; 74 a 51% neutra; e ≤50% frágil. A confiabilidade foi mensurada pelo coeficiente Alfa de Cronbach. Resultados: maior índice foi obtido na dimensão "Expectativas sobre o seu supervisor/chefe e ações promotoras da segurança", com 70,8% (n=455) de respostas positivas e "Respostas não punitivas ao erro" o menor, com 25,6% (n=164). O coeficiente Alfa de Cronbach obteve média de 0,62, mostrando baixa confiabilidade. Conclusão: resultados mostram cultura de segurança insatisfatória e relevância de ações de promoção, com destaque à abordagem coletiva e não punitiva de erros. Safety culture and communication about surgical errors from the perspective of the health teamCultura de segurança e comunicação sobre erros cirúrgicos na perspectiva da equipe de saúde Cultura de la seguridad y comunicación sobre errores quirúrgicos en la perspectiva del equipo de salud ABSTRACT Objective: To analyze the safety culture related to the communication dimensions and event notification from the perception of the health team. Method: Survey carried out in a teaching hospital of Paraná through the application of the Survey on Patient Safety Culture questionnaire to 158 professionals working in surgical units from May to September 2017.The analysis of the data was done by descriptive and analytical statistics; dimensions with positive responses ≥75% represent strong areas for patient safety. Results: No dimension or item/question were considered strong to surgical patient safety. There was a difference, with a less negative perception from nursing in relation to medicine, in the dimensions "Return of the information and communication about the error" and "Frequency of events reports" (p<0.001). Conclusion:The health team perceives that there is fragility in the patient safety in relation to the communication dimension, demanding actions that promote the patient safety. RESUMO: Objetivo: identificar o conhecimento dos profissionais de enfermagem sobre úlceras por pressão no Centro de Terapia Semi-intensiva de um hospital universitário. Método: estudo quantitativo. A coleta de dados ocorreu em dezembro de 2012, por meio do questionário traduzido e validado para a realidade brasileira, com itens de verdadeiro ou falso e desempenho satisfatório igual a 37 acertos (90%). Resultados: auxiliares de enfermagem obtiveram 30,9 acertos (75,33%), técnicos com 28 (68,29%) e enfermeiros com 33,6 acertos (81,95%). Os itens com menos acertos se relacionavam aos cuidados de enfermagem contraindicados: massagem das áreas hiperemiadas, utilização de luvas d'água, inadequado reposicionamento dos pacientes acamados e cadeirantes e ângulo de elevação da cabeceira. Conclusões: os profissionais apresentaram conhecimento insatisfatório, o que demonstra a importância da atualização e capacitação em serviço. ABSTRACT: Objective: to identify the knowledge of nursing professionals on pressure ulcers in the Centre of Semi-intensive Therapy of a university hospital. Method: quantitative study. The data collection occurred in December 2012 and used a questionnaire translated and validated to the Brazilian reality, with items of true or false, and satisfactory performance of 37 items (90%). Results: nursing assistants scored 30.9 (75.33%), technicians 28 (68.29%) and nurses 33.6 (81.95%). The items with fewer scores related to contraindicated nursing care: massage in areas of hyperemia, use of water gloves, inadequate repositioning of bedridden patients and wheelchair users, and elevation angle of the headboard. Conclusion: the professionals presented unsatisfactory knowledge, demonstrating the importance of updating and training professionals in service.
PURPOSETo test the effects of clinical reasoning prompts on students’ clinical judgment of a written case study.METHODSAn experimental pre‐ and posttest study with second semester nursing students (N = 163).FINDINGSThe intervention was insufficient to significantly improve clinical judgment. Students identified that the prompts would help them “narrow… down the problem” and “slow… the decision‐making process” to improve analysis. The most accurate patient problem was identified by 28% of students in pretest and 35% in posttest.CONCLUSIONSThis study provides evidence of variations in nursing students’ clinical judgment and students’ desire to use decision‐making algorithms.NURSING IMPLICATIONSNurse educators should provide students with additional education and practice to identify and solve these types of problems.
RESUMO:Realizou-se um estudo de caso em um hospital de ensino, no período de dezembro de 2012 a fevereiro de 2013, com objetivo de identificar o conhecimento dos profissionais de enfermagem sobre as úlceras por pressão em duas unidades cirúrgicas, com destaque para aspectos relativos à prevenção. Participaram 25 profissionais de enfermagem e utilizou-se um instrumento validado que considera conhecimento adequado aos que obtiverem 90% de acertos ou mais. A média de acertos das unidades foi igual a 73,88%. Os erros mais comuns: o conceito de estágio III das úlceras por pressão e a indicação de massagem em regiões de proeminência óssea, elevação da cama a um ângulo superior de 30º, uso de luvas d'água, almofadas tipo roda d'água, reposicionamento de pacientes sentados. Os resultados evidenciam a necessidade de atualização sobre o tema úlceras por pressão de acordo com os guidelines. DESCRITORES:Cuidados de enfermagem; Assistência à saúde; Úlceras por pressão. KNOWLEDGE OF NURSING PROFESSIONALS REGARDING PRESSURE ULCERS IN TWO SURGICAL UNITS -PART 1ABSTRACT: A case study was undertaken in a teaching hospital, in December 2012 -February 2013, aiming to identify the knowledge of the nursing professionals regarding pressure ulcers in two surgical units, with emphasis on aspects relating to prevention. A total of 25 nursing professionals participated, and a validated instrument was used which considers appropriate knowledge to be held by those who obtained 90% of correct answers or more. The mean for correct answers in the units was equal to 73.88%. The most common errors were: the concept of stage III of pressure ulcers and the indication of massage in regions of bony prominence, raising the bed to an angle greater than 30°, the use of gloves full of water, gel inflatable ring cushion, and the repositioning of seated patients. There is also evidence for the need for updating regarding the issue of pressure ulcers in accordance with the guidelines.
