Wound healing is a complex process that started to be fully understood only in recent years. Recent research has been directed to act in the nutrition modulation of the healing process.
PURPOSE: Analyze the mechanical strength of digestive tract scar after intestinal anastomosis surgery in animals treated with pure Copaíba oil. METHODS: 60 Wistar rats, male, about 250 days old and weighting around 350g were used. The rats were randomly divided into two groups: Group O, with 30 animals that received Copaíba oil and Group C, with 30 animals that received saline. Each group was subdivided into three subgroups, containing 10 rats each. They were designated O7, O14, O28, C7, C14 and C28, according to the post-operative assessment date at 7, 14 and 28 days, respectively. On these dates euthanasia was performed with the removal of the bowel segment containing the anastomosis and assigning the samples to tensile test for assessing Maximum Stress, Maximum Tensile Strength and Maximum Rupture Strength. RESULTS: On the three variables of the study, the results indicate that, for the three assessment periods (7, 14 and 28 days) there was no significant difference between the oil and control groups. CONCLUSION: For the mechanical tests proposed by this study, Copaíba oil didn´t show any effectiveness in increasing the anatomosis strength.
InTRODUÇÃOCicatrização adequada é considerada fundamental para o sucesso de qualquer intervenção cirúrgica, e uma falha neste processo pode levar à deiscências das suturas e graves consequências. A medida em que o processo fisiológico da cicatrização tornou-se progressivamente compreendido, fatores que influenciam este processo foram sendo descobertos e avaliados.Falhas nas anastomoses intestinais resultam em deiscências e fístulas, associadas a alta morbi-mortalidade. Já foram bem identificadas causas locais e sistêmicas que aumentam os riscos de deiscências anstomóticas. Os principais fatores locais são a realização de anastomoses sem tensão, com boa vascularização, ausência de infecção, boa coaptação dos bordos da ferida e inclusão da submucosa nos pontos, por ser esta camada rica em colágeno 9,14 .Além dos locais, a influência de fatores sistêmicos sobre o processo é relevante. A presença de desnutrição, o uso de anti-inflamatórios hormonais e não-hormonais e de anti-neoplásicos e a radiação ionizante influenciam negativamente a cicatrização das anastomoses, enquanto o uso judicioso de nutrição perioperatória, especialmente com dietas imunomoduladoras ou suplementos dietéticos podem aumentar a resistência tênsil das anastomoses 3,5,8,33,39 .Em face à dificuldade em estudar tais mecanismos de ação em condições clínicas, alguns modelos de experimentação foram sendo desenvolvidos. A arginina, aminoácido semi-essencial, possui papel fundamental no metabolismo de diversos componentes do organismo humano 36 . Possui ação importante como substrato na síntese protéica, proliferação celular, neurotransmissão, vasodilatação, imunidade e no processo de cicatrização. InTRODUÇÃOConcomitantemente com a introdução de medicamentos potentes, tecnologia avançada de diagnósticos e novos tratamentos conservadores, muitas operações gastrointestinais tornaram-se desnecessárias, em particular procedimentos cirúrgicos ácido-redutores sobre o estômago. Em contrapartida, as operações sobre o estômago cresceram de forma exponencial com o desenvolvimento da cirurgia bariátrica. Como o tratamento do câncer gástrico permanece sendo fundamentalmente cirúrgico, pode-se inferir que o conhecimento do processo de cicatrização gástrica permanece relevante na prática do cirurgião digestivo.Durante as décadas de 50 e 60, as pesquisas foram direcionadas ao estudo dos fatores de ordem técnica capazes de interferir na cicatrização. Dessa forma, a evolução da Medicina está diretamente ligada à produção de literatura de qualidade e amplamente disponível. A integração entre a Informática e a Medicina tem sido determinante tanto na elaboração de literatura médica, quanto na distribuição da mesma. O uso dos recursos da informática, especialmente no que tange à captura, armazenamento e busca de dados clínicos, tem sido de suma importância na produção de estudos clínicos relevantes e confiáveis 1,2 . Estudos baseados em grandes séries de pacientes fundamentam a Medicina baseada em evidência, contribuem para o estabelecimento de condutas mais apropriadas para as mais diversas doenças, tornando os tratamentos uniformes e melhorando assim os resultados obtidos 3,4 . Dessa maneira, o desenvolvimento de protocolos eletrôni-cos de coleta de dados clínicos capazes de realizar armazenamento estruturado dos mesmos, com objetivo de desenvolvimento de estudos prospectivos, é uma ferramenta extremamente útil na produção de literatura médica de qualidade.No caso da terapia nutricional, o uso da informática e, mais especificamente, o desenvolvimento de programas de computador, tem sido amplamente relatado desde 1975 5 . Desde então, diversos softwares foram produzidos com objetivo de facilitar os cálculos de nutrição parenteral. Esses programas têm demonstrado grande aplicabilidade prática, menor consumo de tempo durante as prescrições e redução nos erros de cálculos dos componentes e na transcrição das mesmas 6 .
