Sehr geehrte Herausgeber, der große, plaqueartige blaue Naevus (LPTBN) ist eine seltene Variante des blauen Naevus, der meist von Geburt an oder in der frühen Kindheit auftritt [ 1 ] . Die Entwicklung eines Melanoms in einem LPTBN ist höchst selten; bisher sind nur wenige Fälle bekannt [ 2 ] . Wir berichten von einem sehr seltenen Fall eines dermalen Melanoms, das in einem kongenitalen LPTBN entstanden ist.Eine 53-jährige Patientin stellte sich mit einem subkutanen Tumor (1,9 × 2,9 cm) innerhalb einer großen blauen Plaque (7,0 × 3,5 cm) an der rechten Gesäßhälfte (Abbildung 1 a) vor. Die Pigmentierung bestand seit der Geburt, der Tumor war ein Jahr vor der klinischen Vorstellung aufgetreten.Die Dermatoskopie der blauen Plaque zeigte sowohl schwarze Punkte als auch schwarze bis blaue Flecken innerhalb blauer homogener Bereiche (Abbildung 1 b). Bei der histopathologischen Untersuchung des subkutanen Tumors stellte man einen gut begrenzten Tumor mit fi bröser Kapsel fest. Im Zentrum der Läsion war eine Nekrosis en masse, an den Rändern Nester atypischer epithelioider Melanozyten zu sehen. Ein Tumornest mit Melaninpigment wurde außerhalb des Tumors beobachtet (Abbildung 2 a). Die übrigen Tumorzellen innerhalb der Nekrosis en masse wiesen nukleare Atypien, Mitosen und zytoplasmatisches Pigment (Abbildung 2 b) auf.
