Background: Wound care with modern dressings that have been widely used takes time to achieve healing. Therefore, Epidermal Growth Factor (EGF) and Platelet-Rich Plasma (PRP) are developed in wound healing therapy. EGF as the primary growth factor examines whether it is sufficient to represent the role of other growth factors in PRP. EGF is also chosen because of its ease of provision and longer shelf life compared to PRP. This study evaluates the (EGF) representing the role of other growth factors contained in PRP. Methods: The study was performed on 66 full-thickness wounds in 6 groups of 36 healthy male Oryctolagus cuniculus rabbits. Four treatment groups were given EGF and PRP therapies. Two control groups were given no treatment. Half of the groups were evaluated on the fifth day and the rest on the fourteenth day. Assessment on the clinical macroscopic and histopathological numbers of fibroblasts, capillary blood vessels, and type III collagen fibers were stained with Hematoxylin Eosin (HE) and Masson's Trichrome. Data were analyzed using SPSS version 23 for Windows.Results: There was a statistically significant difference in fibroplasia (p=0.014; p=0.018) on the fifth and fourteenth days. However, there was no significant difference in angiogenesis (p=0.183; p=0.524) or collagenization (p=0.218; p=0.278) on the fifth and fourteenth day. On the fifth day, the number of capillary vessels was highest in the PRP groups (10.60±4.13), and the ratio of type III collagen fibers (53.00±13.00) was the highest in the EGF group. On the fourteenth day, the number of capillaries and the ratio of type III collagen fibers was the highest in the EGF groups (77.00±16.00). Conclusion: EGF greatly increases the speed of macroscopic healing, accelerates fibroplasia, induces angiogenesis, and is also involved in collagen deposition compared to PRP administration, especially when compared to untreated wounds.
Background Composite graft as a reconstructive therapy option has limitations in size so that it is easily necrotic. Deferoxamine administration has been associated with increased neo-vascularity in wounds. We aimed to compare the administration of deferoxamine and Platelet-Rich Plasma (PRP) injection in a composite graft in rabbits. Methods Thirty New Zealand rabbits were divided into three groups; the control group, the deferoxamine group, and the PRP group. The composite graft with a diameter of 2 cm was taken and replanted after rotating it 180°. The mean graft viability and the mean number of capillaries were evaluated on day 7 (POD 7) by macroscopic and histological evaluation using Hematoxylin-Eosin staining. Results While the mean number of capillaries was not significantly different in control, deferoxamine, and PRP groups ( p = 0.21), the mean survival rate in the control, deferoxamine, and PRP groups reached a significant level with p -value of 0.006 (66.6% vs. 63.8% vs. 99.6%, respectively). Conclusions Deferoxamine group had the highest number of capillaries, but had the lowest survival rate. In the PRP group, it had the lowest number of capillaries, but had the highest survival rate.
Introduction: Cleft lips (CL) and cleft palate (CP) are common congenital diseases caused by gene mutation in embriology development of the lips and mouth at 4 weeks of gestation. CL and CP can cause several problems such as difficulty in eating, speaking, abnormality in teeth and mouth development and ear iritation. Adequate treatment is needed to improve patients’ quality of life. Material and Method: This cross sectional study take samples purposively from all parents who joined in cleft charity surgery in Kencana Hospital, Serang, Banten on October 24th 2018. This study identify parents’ knowledge, attitude and behaviour towards CL and CP and analyzed the relationship of parents’ knowledge and attitude towards CL and CP. Statistical analysis used in this study is Fisher method. Result: Ten patients undergo labioplasty surgery, 1 patient undergo labioplasty revision, 5 patients undergo palatoplasty surgery and 10 patients undergo labioandpalatoplasty surgery. The majority of parents’ education background have income < 3,5 million a month 84.6% (22 people), do not have any social insurance 88.5% (23 people) and joined in because of they cannot pay for medical treatment 96,2% (25 people). 6.2% (25 people) have under knowledge, 69.2% (18 people) have good attitude and 84.6% (22 people) have good behaviour toward CL and CP. There is no significant relationship between parents’ knowledge and attitude toward CL and CP P value = 1,00 (P > 0,05). Discussion: Parent's under knowledge is caused by their low educational background and inaccurate information and education provided by medical professional. But, they had a good attitude and behaviour toward CL and CP because they had initiative to look for medical help and treat their children same as other normal one. Although they received inaccurate information and education about cleft disease, all parents looked for medical help and did what they were told by medical professionals. Conclusion: The majority of parents in this study have under knowledge about cleft lips and cleft palate but they are still looking for medical help to treat their children. This charity significantly helps parents whose children have cleft lips, with or without cleft palate, to get treated in that area.
The rate of failed skin graft in Dr. Soetomo General Academic Hospital Surabaya was around 26% of the cases, and it became a concern because skin graft was the main procedure to close large burn wounds. Many problems might affect this event; one of them was electrolyte imbalance. Hypernatremia was found in major burn patients. This condition disrupted the wound healing process of skin graft. This cross-sectional study evaluated patients with burns admitted to the Burn Centre of Dr. Soetomo Hospital between January 2014 and December 2018. 143 subjects participated in this study. Hypernatremia was found in 16% of all subjects (23 subjects), 19% with hypernatremia (28 subjects), and the majority of it, 65% with normonatremia (92 subjects). This study found that the risk of failed skin graft was higher on hypernatremia than normonatremia subjects. This risk was higher if the skin graft procedure took more than 10%.
Due to issues of limited substances and expensive cost of wound treatment, topical insulin and simvastatin are being developed due to benefits including easy to find and applicable in every health center. Topical insulin can be used in treating diabetic patient’s wound. Simvastatin contributed in wound healing process by increasing angiogenesis and lymphangiogenesis. This experimental study is using randomized post test only control design in male Wistar mice, divided in 5 treatment groups with 9 mice each : carboxymethylcellulose sodium gel, short acting insulin topical gel, long acting insulin topical gel, simvastatin topical 2% gel, and simvastatin 3% topical gel. A full thickness wound was made randomly and observed in 5 days, followed with wound biopsy and analyzing using histopathological specimen measuring the epithelial thickness, fibroblast proliferation and collagen density from each group. Long acting insulin topical gel has the highest average rate of epithelial proliferation and fibroblast proliferation. In simvastatin 3% topical gel has the highest level of collagen density level among other groups. This study showed long acting insulin topical gel significantly has better outcome than other groups.
