O tremor essencial é um dos mais frequentes motivos de consulta ao neurologista e sempre foi considerado uma desordem monosintomática e benigna. Hoje, no entanto, há fortes evidências de que o tremor essencial é uma doença heterogênea e lentamente progressiva, podendo causar importante diminuição da qualidade de vida de alguns pacientes. Pode ocorrer de forma esporádica ou com padrão familiar, até o momento há três loci cromossômicos relacionados ao tremor essencial, são eles: cromossomo 3q13 (ETM1), 2p22 (ETM2) e 6p23. O principal diagnóstico diferencial é o parkinsonismo. As drogas de escolha para o tratamento ainda são o propranolol e a primidona, mas a gabapentina, o topiramato e o alprazolam são opções terapêuticas. A cirurgia está indicada para casos graves e refratários. Neste estudo de revisão buscamos uma atualização sobre os principais aspectos do tremor essencial já que é uma doença comum na prática diária de qualquer médico.
Multiple system atrophy (MSA) is a sporadic, neurodegenerative disorder, clinically characterized by parkinsonian, autonomic, cerebellar and pyramidal signs. We describe two patients showing different presentations of the same disease. The patient on case 1 presents features of MSA-C or olivopontocerebellar atrophy with the pontine "cross sign" on brain MRI. The second case reports a patient presenting MSA-P or striatonigral degeneration and the brain MRI shows lenticular nucleus sign alteration. We think that brain MRI might increase the accuracy diagnostic of MSA.
Introdução. A síndrome de Pisa (pleurototonus) é uma rara distonia truncal tônica que leva à lateralização do tronco com leve rotação do eixo axial para trás sem distonia em outras partes do corpo. Classicamente essa síndrome é relacionada a uma reação adversa aguda ou tardia ao tratamento com neurolépticos mas pode ser associada a muitas outras medicações ou mesmo ser idiopática. Método. Relatamos um caso de síndrome de Pisa em uma mulher de 33 anos como uma distonia tardia secundária ao uso prolongado de haloperidol. Conclusão. Com o relato dessa rara e pouco conhecida síndrome chamamos a atenção para a necessidade do uso criterioso dos neurolépticos.
Introdução. Neste artigo descrevemos um paciente com atrofia de múltiplos sistemas (AMS) na forma cerebelar (AMS-C) que apresenta à ressonância magnética encefálica o “sinal da cruz” tanto no corte axial quanto no coronal. Método. Paciente masculino, 58 anos, há seis anos e seis meses iniciou quadro de ataxia da marcha associada a incontinência urinária e constipação intestinal. Há 2 anos está restrito a cadeira de rodas. Ao exame físico apresentou ataxia cerebelar, rigidez em roda denteada e tremor de repouso bilateral. Resultados. A ressonância magnética realizada após cerca de quatro anos de evolução clínica da doença mostrou “sinal da cruz” perceptível tanto em corte axial quanto em corte coronal. Conclusão. Chamamos a atenção para a importância desse sinal radiológico como auxiliar no diagnóstico clínico da atrofia de múltiplos sistemas.
