Obezitenin prevalansı tüm dünyada ve ülkemizde artmaktadır. Obezite birçok hastalıkla ilişkilendirilmektedir. Bunlardan biride kadınlardaki stres idrar kaçırmadır. Obezite bu hastalığın şiddetini arttırmaktadır. Obezitenin stres idrar kaçırma üzerine kesin etkilerini araştırmak için çalışmalar devam etmektedir.Özellikle pelvik kaslardaki gerilme, stress ve pelvik sinirlerin kronik irritasyonun kadınlarda stres idrar kaçırma etyolojisinde rol oynadığı gösterilmiştir. Tedavide ilk basamak obezite ile mücadeleyi, sonrasında davranış tedavisini ve son olarak gerektiğinde cerrahi müdahaleyi içermektedir. Bu çalışmada kadınlarda obezite ve stres inkontinans ilişkisi güncel literatür eşliğinde incelenecektir.
IntroductionParaurethral cysts may be acquired or congenital pathology, and are rarely encountered in urogynecologic practice. Therefore, no consensus on management of paraurethral cyst could be reached. We aimed to report our experience on the management of paraurethral cysts in adult women.Material and methodsA retrospective chart review was conducted on adult women diagnosed with paraurethral cysts between 2011 and 2012. Patients’ complaints, parity, physical signs, diagnostic tests and the obtained findings, surgical intervention, duration of follow–up period, complications, recurrence and histologic examination of the cysts wall were evaluated in the patients included.ResultsTen adult women, aged 23–48 years (mean: 41 years) with paraurethral cysts between 2011 and 2012 were identified. All patients were multiparous. The patients’ complaints included palpable mass, dyspareunia, and dysuria. All patients underwent preoperative urethrocystoscopy. All the cysts were solitary and in dimension of 1 to 3.5 cm and localized in the distal urethra. All patients underwent surgical excision. The mean follow–up period was 8 months (range: 6–12 months). Neither complications nor recurrences were observed. Histopathological examination showed that 5 cysts were lined with squamous epithelium, 2 were lined with transitional epithelium, and 3 were lined with both types of epithelium.ConclusionsParaurethral cysts may be symptomatic and routine urological examinations are sufficient for diagnosis without using advanced imaging technique. In such symptomatic adult patients, partial surgical resection combined with fulguration can be performed effectively without complications. This treatment modality seems exceptional according to the literature.
Various types of drugs are being used to treat patients with chronic hepatitis B infection. However, these treatment modalities are not without side effects, which may result in decreased patient adherence. Entecavir is an oral reverse transcriptase inhibitor, which is widely used in patients with hepatitis B. Although headache, fatigue and nausea are well-documented side effects of entecavir, its sexual side effects have not been reported yet. We here report on two male patients with chronic hepatitis B infection who reported decreased libido under entecavir treatment.
ÖzetAmaç: Bu çalışmada Sakarya il genelinde 6-15 yaş aralığındaki ilköğretim ço-cuklarında işeme bozukluklarının çeşitliliğini belirlemeyi amaçladık. Gereç ve Yöntem: İl merkezi ve ilçelerde rastgele seçilen sekiz ilköğretim okulundaki 6-15 yaş aralığındaki 3246 öğrenci çalışmaya alındı. Öğrenciler okulda hazır-lanan uygun bir odada kliniğimizde hazırlanmış olan sorgulama formu ile birlikte değerlendirildiler. Bulgular: İşeme bozuklukları formlarının incelenmesi sonucunda toplam 3246 öğrencinin 486%u2019 sında (%15) işeme bozukluğu saptandı. Çalışmaya katılan 3246 öğrencide sıklık sırasına göre fonksiyonel işeme bozukluğu prevalansı %11.2 (365), noktüri prevalansı %8.7 (285), enürezis nokturna prevalansı %8.6 (279), diürnal enürezis prevalansı %1 (32), olarak saptandı. Erkek çocuklarında (toplam 1573) fonksiyonel işeme bozuklukları prevalansı %9 (142), noktüri prevalansı %11.7 (184), enürezis nokturna prevalansı %11.2(176), diürnal enürezis prevalansı %0.5 (8) oranında gözlendi. Kız çocuklarında (1673) fonksiyonel işeme bozuklukları prevalansı %13.4 (223), noktüri prevalansı %6.7 (111), enürezis nokturna prevalansı %6.2 (103), diürnal enürezis prevalansı %1.4 (24) oranında saptandı. İşe-me bozukluğu olan çocuklar ile işeme bozuklukları olmayan çocuklar arasın-da boy, vücut kitle indeksi(VKİ), açısından istatiksel olarak anlamlı fark tespit edilmedi.(p>0.05) Tartışma: Türkiye%u2019den daha önce bildirilen çalışma-larla enürezis nokturna, nokturi, diürnal enürezis prevalansları ile sonuçları-mız benzerlik göstermektedir. Şu ana kadar ülkemizde fonksiyonel işeme bozuklukları prevalansını bildiren bir çalışma yoktur. Fonksiyonel işeme bozuklukları özellikle kız çocuklarında ve ilköğretimin ilk yıllarında sık görülmektedir. Material and Method: Totally 3246, 5-16 years old student randomly selected from 8 schools inside province. Students are evaluated in an appropriate room at school with a form pre-prepared in our clinic. Results: The survey shows that 486 of 3246 students (15%) have voiding problems. The prevalance of dysfunctional voiding problems in all children, was 11.2%(365), nocturia prevalence was 8.7% (279), diurnal enuresis was 1% (32). In boys (totally 1573) the prevalance of dysfunctional voiding problems was 9% (145), nocturia prevalence was 11.7%(184), enuresis nocturia prevalence 11.2% (176), diurnal enuresis was 0.5% (8). In girls (totally 1673) the prevalance of dysfunctional voiding problems was 13.4 (223), nocturia prevalence was 6.7% (111), enuresis nocturia prevalence was 6.2% (103), diurnal enuresis prevalance was 1.4% (24). There was no meaningful statistical difference from the perspective of height and body mass index (BMI) between the chıldren with voiding problems and without any problem (p>0.05) .
