O médico, ao exercer a sua profissão, deve em obediência aos conceitos éticos permeados na sua atividade, zelar e trabalhar pelo perfeito desempenho ético da Medicina e pelo prestígio e bom conceito da profissão. É o guardião da vida, que é o bem maior que o ser humano possui. O médico deve ter dedicação, correção, respeito pela vida e em razão de sua função agir sempre com diligência, cautela e evitar que seu paciente possa ser conduzido ao sofrimento, a dor, a angústia e as perdas irreparáveis. A responsabilidade do médico e os acontecimentos gerados em decorrência de sua profissão podem gerar efeitos na esfera ética, civil e criminal.O médico não pode praticar atos profissionais que possam ser danosos ao paciente, e que podem ser caracterizados como imperícia, imprudência e negligência.A imperícia ocorre quando o médico revela em sua atitude falta ou deficiência de conhecimentos técnicos da profissão. É a falta de observação das normas e despreparo prático necessário para exercer determinada atividade. Devem-se avaliar os progressos científicos que sejam de domínio público, e o profissional deve ter conhecimento para a utilização da técnica indicada para cada tipo de procedimento ou doença. O imperito não sabe, no seu modo de agir, o que um médico deveria saber.A imprudência é a imprevisão do agente em relação às consequências do seu ato ou ação. O profissional médico tem atitudes, não precipitadas, sem ter cautela, sendo resultado da não racionalização. Neste caso, o médico tem perfeito conhecimento do risco, e ignorando a ciência médica, toma a decisão de agir mesmo assim. O imprudente usa terapêuticas sem necessidade e muitas vezes técnicas terapêuticas que podem ser nocivas para o paciente.A negligência acontece pela falta de cuidado ou de precaução com que se executam certos atos. É caracterizada pela inércia, indolência, falta de ação e passividade. É um ato omissivo, oposto da diligência que seria agir com cautela, cuidado e atenção, evitando quaisquer distorções e falhas. Realizaram-se exames clínico-nutricionais e bioquímicos em 109 crianças, de 2 a 7 anos de idade, filhos de migrantes nacionais em trânsito pela Central de Triagem e Encaminhamento (CETREN), na Capital do Estado de São Paulo, Brasil, em que se pesquisaram sinais clínicos de xeroftalmia e níveis sanguíneos de caroteno e vitamina A. As conclusões apontaram existir hipovitaminose A a nível bioquímico, no grupo estudado, não constituindo a xeroftalmia um problema de saúde pública.
Objetivo: Estudar a topografia de córnea após perfurações oculares grau 1. Métodos: Por meio de estudo clínico transversal controlado, foram realizados exames de topografia computadorizada de córnea (topógrafo EyeTech CT-2000) em ambos os olhos em 21 pacientes que haviam sido atendidos e submetidos à correção cirúrgica de perfuração corneana em um dos olhos pela equipe do Pronto Socorro do Departamento de Oftalmologia da Santa Casa de São Paulo no período de janeiro a dezembro de 1998. Os olhos contralaterais formaram o grupo controle. Resultados: Quinze pacientes (71%) apresentaram lesões menores que 4 mm. O astigmatismo topográfico no grupo de olhos perfurados foi 2,66 ± 2,64 D, e 0,52 ± 0,25 D no grupo controle. Ao comparar distribuição dos pacientes com astigmatismo topográfico maior ou menor que 2,00 D nas categorias do tamanho médio e da configuração da lesão, observaram-se diferenças estatisticamente significativas (p=0,04 e p=0,02 respectivamente). A localização não mostrou diferença estatisticamente significativa (p=1,00). Não houve diferença estatisticamente significativa (p=0,98) entre o poder dióptrico corneal de olhos perfurados e do grupo controle. A distribuição, quanto ao padrão topográfico corneal, foi semelhante no grupo de olhos perfurados, grupo controle, bem como na literatura. Melhor acuidade visual corrigida melhor ou igual a 0,5 foi encontrado em 81% dos pacientes. Conclusões: O astigmatismo topográfico resultante de lesão de córnea foi maior que o do grupo controle, porém não houve mudança qualitativa do padrão topográfico destes olhos, excluindo os casos de topografia irregular. Confirma-se a correlação do astigmatismo topográfico com o tamanho da lesão, reafirmando-se o tamanho crítico de 4 mm. Perfurações grau 1 têm bom prognóstico visual, ao passo que pior prognóstico é esperado para padrões topográficos irregulares ou lesões maiores que 4 mm. Corneal topography after corneal perforation
Objectives:To assess the antimicrobial effectiveness of multipurpose solutions in regard to the disinfection of silicone hydrogel contact lenses (CL) using a study of clinical bacterial isolates from ocular material.Methods:Three multipurpose solutions (solution A: polyhexamethylene biguanide 0.00025 g/100 mL; solution B: polyquaternary-1 0.001% and myristamidopropyl dimethylamine 0.0006%; and solution C: polyaminopropyl biguanide 0.00013% and polyquaternary 0.0001%) were used as a 3-phase disinfection on silicone hydrogel CL contaminated with bacteria from clinical isolates that were divided into five groups (group 1: Pseudomonas aeruginosa; group 2: Staphylococcus aureus; group 3: Staphylococcus epidermidis; group 4: Streptococcus spp; and group 5: enterobacteria).Results:No differences were observed between the 24- and 48-hr measurements in any of the samples, and the positivity of microorganisms in T0 was 100% for all solutions; it was 0% in T3. Therefore, only steps T1 (rubbing followed by rinsing) and T2 (rubbing followed by rinsing and immersion of CL into solution) were considered for analysis at the 24-hr measurement time. Throughout the phases, a decrease in the number of bacteria was observed, culminating in the elimination (no recovery) of all microorganisms in the three solutions.Conclusions:At the end of the proposed process, the tested solutions were effective.
