Este trabalho investiga o aspecto qualitativo da Reforma Psiquiátrica no interior do estado de São Paulo, por meio da análise dos discursos presentes no campo da Saúde Mental. Balizamo-nos no referencial da Atenção Psicossocial enquanto corpo teórico-prático e ético para transformação paradigmática em Saúde Mental. Como método, utilizamos entrevistas semi-dirigidas com coordenadores de CAPS e coordenadores municipais de Saúde Mental, e observações participantes, analisadas por meio do método dialético de análise de discurso. Foram encontradas três matrizes teóricas principais dos discursos (Psiquiatria Tradicional, Psiquiatria Preventivo-Comunitária, Atenção Psicossocial) coexistindo no espaço institucional. A vertente Preventivo-Comunitária mostrou-se bem instalada e superando técnica e teoricamente a Psiquiatria Tradicional, porém dificultando o avanço da Atenção Psicossocial em termos éticos.
Resumo O modo de produção de bens materiais é correlato ao modo de produção de saúde e indica as formas de subjetivação possíveis em uma sociedade, portanto, está relacionado às maneiras como impasses sociais e psíquicos se materializam. Os conflitos da luta de classes produzem contradições, de maneira que é crucial observar que os sintomas desencadeadores das crises vêm enunciar objeção ao contexto social no qual emergiram. Partindo da análise organizacional, constatamos que os estabelecimentos institucionais de saúde mental no contexto do modo capitalista de produção têm servido para agenciar essas crises no sentido de dissuadi-las. Observa-se que, apesar dos avanços da reforma psiquiátrica brasileira, os estabelecimentos psicossociais ainda servem à adaptação social, produzindo subjetividades alienadas e reproduzindo formas históricas de dominação-subordinação, como a internação psiquiátrica e a medicalização da vida e do sofrimento.
Analytical listening in the General Hospital: some implications with the analyst's desireResumo:Partindo da experiência na enfermaria de um hospital-escola 1 , do confronto com o status-quo do trabalho dos psicólogos em hospitais e da avaliação da literatura sobre a clínica psicanalítica em hospital, esta pesquisa se propôs a buscar avanços frente a uma questão específica: a escuta psicanalítica no trabalho em enfermarias de hospital geral e as implicações dessa prática com o desejo do psicanalista. Foram entrevistados quinze psicólogos declaradamente de orientação psicanalítica, vinculados a unidades de internação e com experiência mínima de dois anos de trabalho em hospital geral. As entrevistas foram abertas e semi-estruturadas, com o intuito de contemplar de maneira uniforme a coleta de informações de cunho mais objetivo, assim como de facilitar a pertinência ao tema de estudo. A interpretação dos dados foi realizada à luz do referencial clínico psicanalítico (freud-lacaniano), sendo que tal interpretação nos permitiu identificar uma importante interação entre os impasses, presentes na instituição hospitalar, da prática clínica psicanalítica com questões pessoais e de formação do profissional, interação que se vincula à postura de trabalho adotada no hospital. Reafirmamos, através desta pesquisa, a possibilidade de fazer uso de uma escuta analítica no trabalho com pacientes internados, a qual está sempre voltada para as manifestações do inconsciente ao longo da fala, e na qual a direção do tratamento é a emergência do sujeito do inconsciente. Abstract:Starting from the experience with infirmary patients in a school-hospital, from the status-quo of psychologist's work in general hospitals' infirmaries and from literature about psychoanalysis in hospitals, this work is focused in a specific question: the psychoanalytic hearing in infirmaries and the implication of its practice with the analyst's desire. We intervied fifteen hospital psychologists who declared psychoanalytic affiliation and had at least a two year experience in hospitals and work in infirmaries. We use psychoanalytic hearing to do "open", semi-structured interviews. The data were analyzed under a psichoanalytical (FreudianLacanian) framework, and we could identify a strong interaction between the supposed hospital environment difficulties to the psychoanalytical work and some personal and training aspects, and we find that these interactions are linked to the working attitude adopted in the hospital. Our research reinforces the possibility of making use of psychoanalytic hearing in the work with infirmary patients, which is linked to unconscious manifestations along patient's speech, and in which the therapy direction is the emergence of unconscious subject.
