In this article we summarize our own experience of lung cancer diagnostics using exhaled breath analysis with a non-selective method using metal oxide chemoresistor gas sensors with cross-sensitivity combined with the sputum cytology. Volatile organic compounds of exhaled breath change the conductivity of the sensor, the resulting pulse is displayed as a peak on the graph, the area of which is used as test results. The combination of two diagnostic techniques in 204 participants demonstrated the possibility of non-invasively detecting the disease at an early stage. The sensitivity, specificity and accuracy of the breath analysis was 91.2%, 100% and 93.4%, respectively. The combination of the breath test and the sputum cytology compared to the breath test alone showed statistically significant (p = 0.03) increase in sensitivity to 96.8% (95% CI: 80.9% -99%) with acceptable decrease in specificity to 93.4% (95% CI: 88% -96%). The convenience of analysis and realtime measurements show some promise for the early detection.
Аннотация. Вариабельность артериального давления (ВАД) -это диапазон его колебаний за определенный период времени. Она вычисляется как при проведении суточного мониторирования артериального давления (АД), так и путем сравнения по-казателей АД, полученных рутинным методом, внутри и между визитами. В настоящее время в мире активный интерес при-влечен к изучению ВАД в связи с ее доказанной прогностической ценностью в отношении цереброваскулярных и коронарных событий. Также высокая вариабельность АД рассматривается как потенциальный фактор риска развития деменции, в том числе болезни Альцгеймера. На сегодняшний день имеются данные о прогностической ценности ВАД в отношении гипер-тензивных расстройств во время беременности, в частности, преэклампсии. Однако эти сведения нуждаются в подтверждении в дальнейших исследованиях. Также высокая ВАД ассоциирована со статусом менопаузы независимо от возраста. В статье представлен обзор литературы, освещающей тему ВАД в общей и акушерской практике. Выделены основные этапы изучения данной проблемы, представлены результаты важнейших исследований в этой области. Также представлены имеющиеся све-дения о физиологических механизмах, определяющих ВАД как при беременности, так и вне ее. Амплитуду коле-баний АД называют его вариабельностью. Выделяют два типа вариабельности АД: 1) краткосрочную вариа-бельность АД, на протяжении нескольких минут или часов, которую можно определить путем повторных измерений в клинике или с помощью суточного мони-торирования артериального давления (СМАД), и 2) долгосрочную вариабельность АД -от визита к визиту, которую определяют на основании повторных измере-ний с интервалами в несколько недель или месяцев [1].Долгое время ВАД рассматривалась как случайная величина, не заслуживающая внимания при работе с пациентом. Толчком к началу ее изучения послужило внедрение в клиническую практику методики СМАД в семидесятые годы прошлого столетия. Первые статьи принадлежали P. Sleight, G. Pickering, W. Birkehäger, E. Raftery, D.L. Clement. В своих работах они установили связь вариабельности с величиной АД и возрастом па-циента. В это же время начались активные поиски ме-ханизмов, определяющих ВАД [2].На втором этапе изучения данной темы ученые за-дались вопросом наличия связи ВАД с прогнозами для пациента. Два ранних исследования дали старт к началу многолетних поисков. В 2000 году в журнале Circulation вышла статья Dirk Sander и соавторов, по-священная изучению связи ВАД и риска атероскле-роза. Aim. To assess the short-term and long-term variability of blood pressure in women, starting from early pregnancy, to predict the development of complications of gestation, including preeclempsia. Methods. In 131 pregnant women, systolic blood pressure, diastolic blood pressure, as well as short-term (intra-visit) and long-term (inter-visit) blood pressure variability were assessed during the gestation period and 6 weeks after delivery. At the end of gestation period, depending on the identified complications, all study participants were divided into four groups: group 1 - control (healthy); group 2 - pregnant with preeclempsia; group 3 - with placental insufficiency; group 4 - with chronic arterial hypertension. In patients with placental insufficiency the indices of fetal growth retardation were also analyzed. Results. In group 4, starting from the second trimester, higher values of short-term blood pressure variability were demonstrated, which increased as pregnancy progressed. Long-term blood pressure variability increased in groups 2 and 4, starting from the second trimester. In pregnant women with fetal growth retardation in the first trimester, blood pressure variability was higher than in pregnant controls and in the second trimester it was higher than in the group with the subsequent development of preeclempsia. Thus, during pregnancy complicated by preeclempsia or placental insufficiency with fetal growth retardation, high long-term blood pressure variability was observed. At the same time, the highest values were observed in the third trimester. Conclusion. The assessment of long-term blood pressure variability from the early gestation seems to be an effective tool for detecting preclinical changes in the body of a pregnant woman, preceding the development of preeclempsia and fetal growth retardation, and in the presence of risk factors of preeclampsia allows narrowing the group of patients for target follow-up and prevention.
