Editorial Novos maresEntendendo a reestenose -Acreditou-se, por muitos anos, que uma resposta fibroproliferativa exagerada, com resultante espessamento neo-intimal, fosse a causadora do comprometimento do fluxo, na reestenose após o emprego do cateter-balão. Por isso e por mais de uma déca-da, as investigações experimentais e clínicas concentraram-se na prevenção da proliferação celular. Contudo, diferentemente dos muitos modelos animais 1,2 , em artérias humanas, os vários agentes farmacológicos (antiplaquetá-rios, antitrombínicos, antiisquêmicos, antiinflamatórios, antiproliferativos e hipolipemiantes), contra os diversos e complexos alvos fisiopatológicos da seqüência de eventos celulares, pós-injúria com o balão, não foram convincentes na redução da reestenose 3 . As razões para esses resultados instigantes e desapontadores suscitaram especulações, longos debates e investigações, até que, recentemente, com estudo seriado, por meio de ultrasom intracoronário (pós-intervenção e na fase de seguimento de seis meses) 4 , pôde-se entender melhor o processo da perda luminal tardia e interpretar o insucesso das medidas farmacoló-gicas para o controle da reestenose.Permitindo imageamento tomográfico in vivo, pós-intervenções por cateter (que não o implante de stents), esse método pôde subdividir a reestenose em dois componentes básicos: a proliferação de tecido e a remodelação arterial. Mais do que isso, o ultra-som pôde discernir a contribuição ponderal desses componentes, já que 73% da perda luminal tardia deveu-se à remodelação arterial (redução da área de secção transversal compreendida pela membrana elás-tica externa -MEE) e 27%, ao crescimento neointimal (aumento da área de secção transversal compreendendo placa e camada média).Essas verificações foram além. Observou-se que as mudanças na área compreendida pela MEE pós-intervenções percutâneas (excluindose os stents) são bidirecionais. Alguns vasos apresentam remodelação adaptativa ou positiva e outros literalmente "encolhem", o que foi chamado de remodelação patológica ou negativa.
