Le syndrome de Moymoya est une anomalie de l’angiogénèse, souvent responsable d’accidents vasculaires cérébraux par sténose progressive des artères de la base du cerveau. C’est la portion intracérébrale de la carotide interne qui est concernée ; d’où le développement d’un réseau collatéral de suppléance qui donnera l’aspect de « fumée de cigarette ». Les manifestations ischémiques sont fréquentes chez les enfants et les adultes mais les atteintes hémorragiques semblent plus fréquentes chez l’adulte et ne représentent que 10% des atteintes pédiatriques. Le pronostic peut être assombri par le risque de récidives mettant en jeu le pronostic neuro-intellectuel et même vital. Le diagnostic repose sur l’angiographie qui représente l’examen de référence alors que l’angio-IRM peut la remplacer si les lésions sont typiques. Le syndrome de Moyamoya peut être primitif ou secondaire à une autre pathologie dont la recherche étiologique reste primordiale. Nous allons décrire un cas de Moyamoya chez un jeune homme découvert à l’occasion d’AVC ischémiques à répétition dont l’origine toxique semble la plus probable.
